Students registering for an athletic team must have a current sports phyiscal on file in Skyward. Student's without a valid physical may not participate in practices or tryouts. To ensure your student is cleared for participation, physicals should be completed by a physician and a copy turned into the Activities Office at least 3 days prior to the first day of practice or tryouts. 

Sports Physicals are valid for 3 years from the exam date, and a copy must be on file in Skyward. Return completed physicals to the Activities Office. MSHSL 2024-2025 Sports Physical Form
